Testosterone therapy, explained without the hype

Low testosterone is real and treatable — but it’s also over-marketed. TRTUSA lays out what the research actually shows, who genuinely benefits, what the risks are, and how to get tested properly before anyone writes a prescription.

<300 ng/dL — threshold many guidelines use for low T2
2–6 wk before libido & energy effects appear3
3–6 mo for muscle & body-composition change3
Lifelong therapy is typically ongoing once started
Context that gets left out: testosterone naturally declines about 1–2% per year after age 30. A below-average number for your age is not automatically a deficiency that needs treating. Symptoms plus confirmed low labs — both — are what matter.1,2

🧪 “My T is 380, my clinic says I need TRT.” 380 ng/dL is within the normal range. A number below average isn’t a deficiency. Symptoms + two morning tests below ~300 is the bar. Get a second opinion.

Before you start TRT, ask about fertility. Exogenous testosterone suppresses your own production and can drop sperm counts to zero. Reversible for many, not all. If you want kids, there are alternatives — discuss them first.

TRAVERSE (2023, NEJM): in men with low T and high CV risk, testosterone was non-inferior to placebo for major cardiac events. Reassuring — but it studied treatment of deficiency, not boosting normal levels. Don’t over-read it.

Understand it first. Then decide.

TRT can be genuinely life-changing for men with real deficiency — and a waste of money and risk for men without it. The difference is a proper workup.
